Developmental Disabilities (DD)
BrightPath is well-equipped to assist individuals with developmental disabilities in accessing and benefiting from the Developmental Disabilities (DD) Waiver in Minnesota. Our services align with the goals of the DD Waiver, ensuring that individuals receive comprehensive, personalized support that promotes independence and community integration.
About the waiver:
The Developmental Disabilities (DD) Waiver in Minnesota is a Medicaid program designed to support individuals with developmental disabilities who require the level of care typically provided in an intermediate care facility for individuals with developmental disabilities (ICF/DD) but prefer to live in their own homes or community settings. The DD Waiver provides a variety of services that promote independence, self-sufficiency, and community inclusion, enabling individuals to lead fulfilling lives outside of institutional settings.
The DD Waiver covers a broad range of services, including:
Case Management
Residential Services
Day Programs
Employment Services
Personal Care Assistance (PCA)
Home and Vehicle Modifications
Respite Care
Behavioral Support
Assistive Technology
Eligibility:
To qualify for the DD Waiver, an individual must:
Have a documented developmental disability or related condition that manifests before the age of 22 and is expected to continue indefinitely.
Require the level of care provided in an ICF/DD.
Be eligible for Medical Assistance (MA).
Choose to receive services in a community setting rather than an institutional facility.
BrightPath embraces a person-centered approach, which is fundamental to the DD Waiver. We work closely with individuals, their families, and support teams to develop customized care plans that reflect each person’s unique needs, preferences, and goals. This approach ensures that the services provided are tailored specifically to enhance their quality of life.
By choosing BrightPath, individuals can confidently access the services and support they need through the DD Waiver, knowing they have a dedicated partner committed to their success and well-being.
