About the waiver:

The Developmental Disabilities (DD) Waiver in Minnesota is a Medicaid program designed to support individuals with developmental disabilities who require the level of care typically provided in an intermediate care facility for individuals with developmental disabilities (ICF/DD) but prefer to live in their own homes or community settings. The DD Waiver provides a variety of services that promote independence, self-sufficiency, and community inclusion, enabling individuals to lead fulfilling lives outside of institutional settings.

The DD Waiver covers a broad range of services, including:

Eligibility:
To qualify for the DD Waiver, an individual must:

  • Have a documented developmental disability or related condition that manifests before the age of 22 and is expected to continue indefinitely.

  • Require the level of care provided in an ICF/DD.

  • Be eligible for Medical Assistance (MA).

  • Choose to receive services in a community setting rather than an institutional facility.

Megan, who is transitioning from a higher level of supervision to independent living, contacts 24hr Emergency Assistance. The leadership team, understanding her unique needs, offers immediate assistance, connects her with disability services, and coordinates resources to address her concerns effectively during this transition period.
