The CAC Waiver provides services that help individuals achieve greater independence and quality of life while staying in their homes or other community-based settings.

Eligibility:
To qualify for the CAC Waiver, an individual must:
Have a severe and persistent disability or medical condition that requires a level of care usually provided in a hospital or skilled nursing facility.
Be eligible for Medical Assistance (MA).
Choose to receive services in a community setting rather than an institutional facility.
