About the waiver:

The Community Alternative Care (CAC) Waiver in Minnesota is a Medicaid program designed to support individuals with severe and persistent disabilities who require the level of care typically provided in a hospital or skilled nursing facility but prefer to live in community settings. The CAC Waiver provides services that help individuals achieve greater independence and quality of life while staying in their homes or other community-based settings.

The CAC Waiver covers a range of services designed to support individuals in their community living arrangements, including:

  • Case Management

  • Residential Services

  • Personal Care Assistance (PCA)

  • Home Health Services

  • Home and Vehicle Modifications

  • Assistive Technology

  • Respite Care

  • Behavioral Support

  • Consumer-Directed Community Supports

Eligibility: 

To qualify for the CAC Waiver, an individual must:

  • Have a severe and persistent disability or medical condition that requires a level of care usually provided in a hospital or skilled nursing facility.

  • Be eligible for Medical Assistance (MA).

  • Choose to receive services in a community setting rather than an institutional facility.

Aging in Place Assistance:

Mr. Thompson, an elderly individual living independently, encounters a change in his mobility and reaches out to 24hr Emergency Assistance. The team, familiar with his health history and living situation, provides immediate support and coordinates necessary services to help Mr. Thompson stay as independent as possible in his own home, ensuring his well-being and safety.
