About the waiver:

Community Access for Disability Inclusion (CADI) is a Medicaid waiver program in Minnesota designed to support individuals with disabilities who require a level of care typically provided in a nursing facility but who prefer to live in their own homes or community settings. The CADI Waiver offers a variety of services aimed at promoting independence, inclusion, and community integration for individuals with physical, intellectual, or developmental disabilities.

The CADI Waiver covers a broad range of services, including:

  • Case management

  • Personal care assistance

  • Home modifications

  • Respite care

  • Employment support

  • Adult day care

  • Specialized equipment and supplies

  • Behavioral support

Eligibility: 

To qualify, individuals must be under 65 years old, require a nursing facility level of care, and be eligible for Medical Assistance (MA). They also must choose to live in a community setting rather than an institutional setting.
